WHAT: The Mercy Consultations (Clinic) is to get a Doctors Signature for People who Qualify for the Oregon Medical Marijuana Program (OMMP), enabled by ...

WHY: to Service those in the Salem area to Help Get Their Card -or- to Re-New; to Help Folks Qualify for the OMMP who are having difficulty;
to Get People (Registered) With The (OMMP) Program in general.

WHEN: Starting Friday, July 23rd, (or thereabouts), 2010
and every Friday after that ... as long as we have enuff Patients.
HOW: Make contact and start the process of (a) transferring records to MERCY Consultations -or- (b) obtaining them {as the case may be} by -
(1) VISIT. Applying at the MERCY Office, at – 1469 Capital Street NE, Suite #100, Salem, 97301 - call 503.363-4588 for more,

Have Records Faxed to the MERCY Office at - 503.581-1937 - and please include contact info on the cover sheet -or- follow-up with a call to confirm.
Consultation (Clinic) Process

Getting and Sending Records.
| Get current records, Chart Notes and the like, especially those regarding your Condition/s that you are going to use cannabis (marijuana) for.

Get them to MERCY. Bring them into the Office, or have your regular, primary care physician or clinic Fax them to us.

Physicians Assistant Review and Setting an Appointment.
The Physicians Assistant will review the records and get back to you to either to set up an appointment or discuss what more is needed.

Consultation (Clinic) Facts

Clinic -vs- Consultation, Long-term Plans.
| We are calling this a Consultation because a Clinic, officially, requires more medical equipment, and the like.
In the long run we want to use this as a basis to start a full service clinic to meet all the needs of all the community.
Physician is Medical Cannabis (Marijuana) Specialist and NOT Primary Care Physician.
| Doctor will not be treating patients as Primary Care Physician, nor prescribing. We can not, currently, utilize this doctor to Qualify Patients Conditions.
We'll be networking Patients to local clinics and doctors to perform this service as needed until MERCY can fulfil this need as well.

What Will It Cost? | The cost will be $200 for new Patients and for ReNewals.
SORRY! We do not have a sliding scale, or payment plans for those financially strapped at this time ... we need to get this thing off the ground 1st!

- we are not accepting Credit Cards (yet, machine and service planned ...) Apologies for any inconvenience and difficulties.
What If One Does Not Have a Primary Care Physician, or Otherwise Does Not Have Current Records?

What If One Does Not Have Insurance?

We have yet to hear of Insurance Company that will pay for Getting Your Card. If one needs a Local clinic or Doctor to perform Primary Care Physician duties and update records to Qualify Condition, one will have to come up with the money.
